Service Requests

All requests for Tenant Data Services moves, adds, and changes must be filled out on this form.  Charges listed are one-time connection fees to activate and/or add additional data ports.  Monthly fees for use of data ports may apply. Each company is allowed one port included with the basic service, each additional port is $15/mth.  Allow 2 days response times for Activations and 5 days for Adds.  We will send you additional IP addresses if needed.

 (We will verify location with contact person before installation.)

De-Activate existing live ports.
 (No charge for de-activations but base monthly useage fees will be charged through the end of the month.)
